You have to use the GM_SetMiscOption function with the GM_MiscOpt_LinearMeasureUnits flag and a value of GM_LINEAR_METRIC to set the linear measure units (such as for the scale) to metric.

The test API code in the sample application demonstrates offsetting the elevation values in a loaded elevation layer. In release code you won't see that. However as others have asked the same thing I'll remove that sample code as it causes confusion.
